Following is a collection of links that can be helpful in conducting your research. They are current as of the revision date above. Most forms are downloadable in both Microsoft Word and Adobe PDF format. Most of the PDF forms have the capability to be filled out electronically; however, note that you will not be able to save these PDF forms as a filled form unless you have the full Adobe Acrobat application, as opposed to Acrobat Reader only.

Guidelines

Vice Chancellor for Research
The vice chancellor for research is the senior campus officer with responsibility for advancing research projects at the University. The vice chancellor's staff works with other Illinois college deans, department heads, faculty and graduate students, as well as external agencies and groups, on behalf of the University's research and graduate programs.

Institutional Review Board
All research involving human subjects that is conducted at or sponsored by the University of Illinois at Urbana-Champaign, whether funded or unfunded and whether conducted by UI faculty or others, must comply with applicable policies for the protection of human subjects. The IRB office, while performing administrative functions of the IRB, also serves as the official oversight office for human subjects research.

IRB-1 Application Tipsheet
All human subjects research projects must be reviewed and approved before human subjects are recruited or involved in research activities. All projects, regardless of exemption status or the type of review they are given, must be presented on an IRB.

  • Waiver of informed consent
    Please see the conditions that must apply if the researcher requests a waiver of active, informed consent.
  • School research form
    The Bureau of Education Research also has a number forms and applications that must be used when conducting research that will take place in an elementary, middle or high school setting.

  • Research project status report
    This report must be used to provide a summary for school districts and other organizations at the end of the academic semester or at other times when a status report is required.

  • Background check
